Transgender is a term used for people who do not fit into the traditional male or female gender. It can include people who are born "intersex" i.e, with both gender's sex organs or with indeterminate sex organs and/or whose DNA isn't strictly male or female; people undergoing sex change operation; or people who simply do not feel that their body is the right gender like in "Boys Don't Cry." It's a term that was developed to encompass all these situations because we didn't used to have a term in the English language to refer to them by. Thanks for asking!

They say that something happens when the baby is being developed and the brain will think that its a female even though it has the organs of a man or vice versa. So as young as 3 the child feels like a girl only has man parts.

Briefly a transvestite is a male who likes to wear women's clothes but is otherwise secure in his maleness.
A transgender person is a person who wants to some extent live in the role of the opposite sex but not necessarily change the external sexual characteristics of his or her body.
A transsexual person is a person who feels that in order to get on with his/her life he/she needs to change the sexual characteristics of his/her body so that they conform to the person's own gender identification.
The term transgender is popularly used as a catch-all for all these categories.
Transgender says nothing about the person's sexuality.
Many transgenders start out by being heterosexual, although for various reasons many have a low libido.
