It shows at the end of Saw crawling off without a foot, in the third Saw it shows what happened to everyone, but him. Did I miss something?

Yah, once he cut off his own foot he was preety much dead already. You can't live that long after doing that cuz you'll lose way too much blood. So when he crawled out of the door he probably died a few yards later. That's why the other guy broke his foot so that it could slide through the chain so he wouldn't lose that much blood.
