My program is for six months and after six months if the student wants to continue then they need to renew the contract. I charge $80.00 a month for an hour of class each week.

Okay!
1. You need to have a form that icludes personal information.
2. You need to have a signed contract.
3. Figure out the rules: If you don't come. Canceling. Any make up days?
4. Make a contract that icludes the charge or fee, how long the class is for, when it is and any other information regarding the class.
5. You need to have a receipt that iclude the person's name, date, cost and your name.

Just be basic, don't scare you potential customers away with some lengthly contract. State the facts.
Example:
This contract is for six months, after which another contract will need to be signed which is good for another 6 months.
The monthly charge for services rendered is $80.00 per month.
If you break the contract there will be a $XX.XX fee.
No refunds if you are a no call no show.
Daily % reduced for illness.

Or something on that order.
Also, maybe contact an attorney.

Be sure to include that the student pays whether they show up or not and any details regarding getting out of the contract early, like paying half of the remainder of the contract or something.

Just what you said but you need to put in something about what happens

1) If your or your student is sick
2) If they move
3) If they decide to cancel for any reason refund policy
4) If they are late
5) Bad weather
6) If you have to cancel
