Are you talking about the telephone excise tax credit? If you file a return, there's a line on the 1040, 1040A and 1040EZ to enter the credit amount. It's down near where you enter your amount of taxes withheld. The credit is between $30 and $60, depending on your family size.
If you don't file a tax return because you don't have enough income to have to, but are still eligible to claim the credit, then there's a form 1040EZ-T that you can use to just file for the credit. You can download it at irs.gov

I spoke with Verizon regarding this already. They refused to give me any tax refund and said they have no duty to do so. I checked the IRS website as well as my Turbo Tax software; I was unable to find any law that requires Verizon to refund the tax. Instead, both Verizon and the IRS said you can get a refund on your 2006 income taxes.
For a single person, you get a standard credit of $30 (not a deduction but a CREDIT - which is better). Alternatively, you can add up the taxes you paid 2/1/2003 to 7/31/2006 and get a credit for that amount instead of the $30. The problem is that Verizon will charge you $6/statement to view your old statements and it's not worth it.

Your refund will show up on your 2006 tax return if you prepare it properly. If you are unsure how to prepare your return, you should have a professional do it.
