Well, actually, a wormhole is nothing more than a mathematical figure. It exists only in theory. It is hypothesized that it forms when spacetime is folded. You can learn a lot more from this link. I have found that wikipedia is a very good source of information. Hope this helped.

Well on earth worm holes are formed by worms. they do that in a garden.
According to the imagination of Physics a wormhole bears similarity to what they called the Einstein -rosen Bridge; where spacetime is folded and you can go from one side of spacetime to the other without any time relativity.
we cant really say if they exist if we have never seen one.Let alone its formation. imagination is a great gift for developing new theories.
Perhaps String theory may offer an explanation for this in the Near future. However Relativity theory still predominates.
