Most are formed when a massive stars basically expends most of its nuclear fuel and collapses inward because of gravity ..

it is difficult to say because blackholes are assumed to have formed. However ;no One has seen one being formed.
Basically it would be very interesting to see a black hole.

Those are just theories, i just think we know about blackholes the same that europeans knew about the earth shape before columbus. We are not advanced yet to know exactly what a blackhole is and how its formed. We can only speculate.
