If you’re talking about a LEGAL annulment (instead of a religious annulment, which I know nothing about) then it can be done without the spouse’s signature. It’s the JUDGE that grants the annulment, not the spouse. Of course, the spouse does have to be notified and they are given a chance to contest it. AND you have to meet the grounds for annulment.

Of course. You cannot just decide to annul your marriage without the other one's consent.

To my knowledge yes. If the person is deceased, or if they are missing, then you probably can obtain an annulment as well.
