I don't know about the entire world, but in the US I'd say New York City has the most gays by far.
NYC is 10 times the population of San Fran (7 million to 750,000) so it's only logical there are a lot more gays there.

Cities like New York City, Los Angeles, Miami, Toronto, London, Madrid, Amsterdam, and Berlin have just as many (or more) openly gay people per capita as San Francisco.
San Francisco has the reputation because it was the first city in the world to welcome gay people. But that was in the 1960s (when homosexuality was a felony in most of the Western world). As more cities in Europe and North America become queer tolerant, however, San Francisco is no longer predominant. It's just one of many places gays can go.

It is definitely San Fran, I have been to New York, Philadelphia, and other major cities. Their population is very visible and accepted. There are gays there but not like San Fran. They are every where. Not that it is anything wrong with it. To each its own.
