Using too much, using the wrong color and applying it incorrectly. You should not "smear" or "wipe" it on your face like you are applying icing to a cake. First, you should put a dot of foundation on each point of your T-zone. If you want a more natural look, use a tinted moisturizer or add a dab of moisturizer to the foundation so it doesn't look like smearing paint- it will be more translucent while still giving you adequate coverage. Put a little foundation on a semi-moist sponge (run under water and then squeeze out) with a dab of moisturizer. Then dab it from from your chin upward (against the growth of the hair). If you apply it from the forehead down, you end up having that "line" and the foundation sticks to the hairs-not your skin. Also, be sure to blend it into your neck, not just the bottom of your chin and jawline. This goes for the ears as well- dab your sponge onto your ear after you have applied it everywhere else (when there is little foundation left) so it blends better.

Besides everything listed above, people make the mistake of using sponges which absorb most of your product so you have to go back and buy it again twice as fast, they also harvest bacteria and in some cases leave the sponge on top of their foundation and reuse it everytime which puts dirt and bacteria in the makeup and back on your skin over and over again. Some women that use a powder foundation will notice their powder darkening and hardening in the case, this is why. Also, using their fingers which puts bacteria right back into the makeup and onto your face as well. Investing in a good quality foundation brush will allow for better application and a good powder type brush is excellent for blending and being sanitary. They should be washed after every use with a brush cleaner. :)

When i pick out foundation i always pick out two colors, one that is my normal facial and neck color and one the next shade darker. The reason for this is so that i can blend a few drops of each and have a desired look that isnt too pale , like me, or way to dark like the other would be alone. I also use a spong applicator to kind of dab it on instead of rubbing it in and leaving smear lines. I usually dab a little along my face/neck line too to help a natural look appear. Next I use a brush and blend in my facial powder which is a mixture of a light color for my skintone and a bronzer to help give me a natural glow. The key to good foundation is to look natural and not like a manikin in a department store. When you have the right foundation and face look you can get away with a lot less make up. You can use a light blush, mascara and lip gloss. This is a great summer look. In winter step it up a notch with some eyeshadow and lipstick.

Definately piling it on too thickly, it's too obvious and not good for the skin. It's not necessary to use a completely covering foundation in most cases.
Also, the classic thing is colour mistakes- it's a good idea to try the foundation on before you buy it, on your face or close to it, and preferably in daylight.
Another one I can think of not applying it well enough in the various creases of the face, thus getting white lines, or not applying to the eyelids or the edge of the face towards the neck. Blending is a key factor in foundation application, and also topping it up with powder. I use a blush brush to apply powder, so I don't make the whole face too powdery.

Too thick, not blended, make-up line, wrong mixture for their skin type (pore clogging or irritating to their skin), wrong coloring - too dark, too light, to yellow, to pink etc... Another problem is not removing it well enough with a good wash, exfoliation, toner, and replenishing with a good moisturizer. Foundation looks best if someone takes good care of their skin! Bad skin reflects poor application no matter how hard you try!

The most common mistake I see is that they fail to blend beyond the jaw line, ito the neck area. The second mistake I notice; they use the wrong shade for their skin/hair coloring.
Hope this helps.
