You don't. Requests for raises should be verbal. Ask to meet with your boss to discuss your performance. Be sure to list your accomplishments that are above & beyond what is called for in your job. Just doing your job is no reason for a raise.

weigh ur +ves and work ur worth to the company.
in ur mind work on some of the real time situations that u worked for company's betterment
think of situatioms when ur commitment and effort benefitted the company.
think of the situation when u worked as a team player and helped the other team members
think of the situation when u helped resolve the clients problems.
now get the confidence in urself that u are worth the increment that u are going to talk about.
and set the appointment with ur boss and while in the conversation talk about the situations ( when asked for/ or when suitable) where u helped the company to grow.

talk for increment is also like an interview so it is verbal communication, not written.

To write a letter of request would probably be too formal for you to do. First of all make your boss feel at home with your presence or to let him feel that he is important in your life's dealing. Invite him for lunch or dinner or for a party in your home or other significant places. Establish first that he is your friend and that you are worth more than your work. Of course you have to show that you are dedicated and trustworthy in your own field and also establish good and friendly relationship with your colleagues. Of course friendly dialog will help!

Do it in person, it will impress your boss more. And don't bother him with stuff like. I've got too many bills, I'm trying to put my kids through school, their going to foreclose on my home. Just tell him how you have improved in your job and how you plan on improving and becoming more productive. This should make your boss happy and want to give you a raise.

Don't. Ask for a raise in a face to face meeting. Request a meeting, prepare what you are going to say, and go for it.

You don't! You do that verbally, in person. Only! You need to prove your value to the company and show that you are worth more than you are being paid. Just listing accomplishments won't cut it.
