Over the phone:
"I'm calling about your opening for blablah who may I speak to about that?"
Or, "Hi, regarding your ad for blabla, would you please tell me more about it?"
Ask if you should apply online or pick up an application in person.
In person:
"Hello. I'm so&so, I'd like to apply for the position of blabla, may I have an application?" Also ask: "what is your deadline for hiring/by what date will you decide who you're hiring?"
You can take the app home and bring it back when completed, but if possible, stay there and fill it out (bring a black pen & clipboard).

first if you have the address for the job, you should go instead of calling, unless the ad specified a call for information.
as a person who does the hiring in the business, i am less than thrilled when i get a person who calls to get the information about the job. it seems like they are calling to see if the job is "good enough" for them before they bother to come and begin the application process. honestly if they confirm that they called first befor coming in, i take extra time to review their application and double check their references befor considering them for employment. in my expienence if they can't bother to do what it takes to make a good impression to get the job, then they do not normally have a good work ethic and usually call out more than others, or usually have major drama in their lives that they think will be a good enough excuse for not working that day. so i am less likely to hire them over someone who came to get the information about the job, instead of taking the lazy way out and making the phone call to see if they even wanted the job, like it is really up to them if they get the job or not.
long and short of it, if they can make the effort to come and get the information about the job, then i feel they will likely go the extra mile to get their job done and will care about the job they do. the person who calls for the information will likely take the easy way out everytime, do the bare minimum to keep their job, and will try to take every short cut imaginable. now which catagory would you like to be placed in?

say what you have to say.. u really like the job and u'll tell em ur work history and what you can do for that position. and arrange the intenview, make it short and sweet on the first call so u don;t give too much away, thats what u do in the interview...

Just call and tell them you're interested in applying for the position. They'll tell you if they want you to come fill out an application or e-mail your resume or fax it or whatever.

Learn a little background on the company (eg, mission statement) and when you apply, mention that you have valuable abilities or experience to offer them that relates to it, and you would like to apply.
