Water pass through a tank filled with catalytic resin that absorbs the minerals in the water. The resin is then washed with a brine solution (salt water) the brine leaches the minerals from the resin beads and is rinsed away. Salts leaching quality is the reason you are told not to consume excessive amount of salt. It leaches the minerals from you body. Low potassium and magnesium will cause high blood pressure.

I happen to work work a Retail store that sells water softener. Hard water means that the water has various calicum salts and other minerals that binds with soap molecules creating a film. This soap film binds to the walls, skin, etc. making walls look grimy and people's skin feel itchy. The softener is filled with table salt (NaCl ) or potassium chloride (KCl). The water going through the softener does an ion exchange binds with the calcium and other minerals. Thus the water has less hard minerals, and the soap is more sudsy and less likely to create soap film.
