Yes, its an island, more like a main island and a few small islands, the causeway was built later.

I've seen the causeway and one of the smaller islands from a plane. It looks amazing!

There's a causeway connecting it to Saudi Arabia (you can see from the plane if you're flying to Bahrain, Qatar). I've never been on the causeway but it looks cool from the air.
