Put your college address on your resume. Do not put anything about the change after a certain date, since it is unnecessary and will clutter your resume. If you do move before you have any response from the company, then fillout a change of address form from the Post Office and they will forward your mail.

On your resume, you should add the address where you will be residing while employeed with that company. Most respectable companies will send out a letter of invitation for the interview. It would be wise to include one where you will get that information. Thank you and good luck.

I would put the college address and contact number on the left, and then I would put: "Contact After XXXdate" as the heading for your parents home address.

Good luck!
