I can't say anything about the number of scoops -or the sizes of each. But the matter of putting chocolate syrup on it is something else. Some things automatically get sauce...spaghetti, mashed potatoes (gravy), and a few others come to mind. But if the restaurant smothered my steak with A1 Sauce, without asking me, I'd send it back! When you order pie, the server will usually ask if you'd like that alaMode.
Unless you ordered a Sundae or something, they have no business putting chocolate or any other sauce on it. For all they know, you could have an allergy to chocolate. If you didn't want or ask for the sauce, then I'd say give 'em hell! No, you should NOT have to specifically state that you don't want something that's not even on the menu. You don't have to state that you want it in a bowl and not a cone. I really hate when servers assume they know what I want or don't want.

Not many people only eat one scoop.. if you think of any desert that is served at the restaurant it probably has HUGE portions(cut a whole pie into like 4 slices or something). Thats why they charge so much. Some places will charge by the scoop, so I would specify then. But if it's ll the same price, order the bigger portion anyway and perhaps share it!! At least your not at King's, they have a desert that has 24 scoops of ice cream!!

Does anyone really want only one scoop? That sounds little to me. I'd be happy with three, but then again we eat lots of ice-cream in my country.
If you don't want anything extra then it's probably better to mention that. Just ask for a vanilla ice-cream with no sauce or anything extra on it. If you want really little you can mention that too (but note that they will likely charge you the full price anyway).

Be specific and if they send it out with chocolate sauce, send it back and tell them that is not what I ordered. Also for smaller portions, order off the children's menu, that's what I do when it comes to dessert.

3 scoops sounds pretty good to me!
I love it with chocolate sauce!
BUT!
If it's not what you ordered and you don't like it - tell them.
