Eucerin has helped my daughter's excema & was recommended by our pharmacist. Also, A&D ointment works well (the gel kind, not the cream kind with zic), and actually Bag Balm is excellent for super-dry areas.
Avoid any baby creams with zinc oxide and don't use as much soap on her. Try every other bath just putting baby oil in the tub with her (a very little amount) instead of using soap.
Think about how silly we are to constantly scrub all the natural oils out of our skin & then put back unnatural chemical oils in place of them. She needs the bath tubbies for rinsing because of diapers & whatnot, but she doesn't need so much soap scrubbing!

Usually it is from soap and water air drying back there. What I did was do the lotion thing, but also whenever I bathed my daughter, I made sure to do some extra rinsing around the ears, and then dry there with a towel specifically, and then comb the hair away from being behind her ears. It did help a lot.

Lotion is not enough. You need a thick ointment cream like Eucerin or Aquaphor. This forms a barrier over the skin so the moisture is locked in, it will not get irritated, and will heal.

my youngest son has had the same thing, we just made sure to wash behind his ears with a wash cloth and mild baby soap at each bath as well as using moisturizer a couple times a day, during the winter months. Not sure what it is, but once the cracks appear it takes about 5-7 days for it to clear up, with daily attention, blessings to you and your little one.

Could it be cradle cap? My girlfriend's son has had cradle cap behind his ears since he was a newborn. Cradle cap is best helped by shampooing thoroughly and taking a comb to remove the scales. I always used Newborn Shampoo by Mustela. You can buy it on amazon. It cured by son's and it's not come back...he's 10 months old now.

My 9 month old has the same thing on her neck. We are using Eucerin which has helped a little, as well as baby oil.

My daughter is older and I've noticed that. I used some baby oil on it before her bath, and be sure to wash and rinse and dry it good afterwards.

find a new doc for her and see what they say it is it sounds like to its the same thing the troops are dealing with in Iraq ask the doc about that to and see if it is that ok
