Yes, the friction material can be worn off from too much slipping or even burn to a hard smooth surface from too much heat as a result of slippage. Can happen from slipping too long before full engagement and/or driving with foot on clutch pedal at all times letting some un-noticeable slipping at all times.

Yes. I attempted to teach a friend to drive a car with a clutch, my car with a fairly new clutch, and burnt the clutch out in one day.
