To assign sex, do an ultrsonographic scan and laparatomy if needed to see the internal genitalia. Ultimately, sex can be assigned by looking at the chromosomes, whether they are XX or XY. The sex of rearing is assigned after keeping in mind the psyche of the person, whether the person can be a functional male or female and the parental wishes. Hormone treatment and surgery has to be done.

People can be assigned a sex, as in the past, or genetically tested for a determination. Quite a few of the world's flora and fauna, of course, are asexual and have no actual gender, though there's no telling the grammatic gender.

you would look at the sex chromosomes. if they are XY then he is male, if XX she is female. sometimes there are 3 instead of 2 (XXY- male, or XYY -super male, XXX- female). usually any extra X will be inactive.
in some animals, fruit fly, it is not the presence of the Y that determines maleness but the absence of a second X.
