No one can give you a serious reply without a check up. Its best your friend sees a gynecologist who will check her and see why she was bleeding.

she should get it checked anyways to make sure not an infection of any form and any bacteria germs dirt etc cause when person masturbates if hands not washed or forgot to be washed before and after masturbation can cause infections from masturbation and also when if a period or something else with vaginal bleeding can get dirt germs infections from any bllod anyone elses and selves .

It is possible either one: virginity loss or period, I hope she is using her toys properly and that they are appropiate in size. If you are the partner instead of a toy maybe you need to prepare her and be gentle. Talk to her. She will be the one to give you the best feed back.
