the pain usually begins in the middle of the night and causes him to vomit. he is unable to lay down in bed and it lasts for several hours. sometimes he will have diareah for a day or two before these symptoms begin

There are lots of potential reasons for this which can not be diagnosed over the internet! everything from cancer to infection to anxiety. Do him and yourself a favor and go to a real doctor, before you have worsening of a real problem! Don't rely on internet opinions for this!
I am a physician and I wouldn't begin to risk a guess to what may be causing this without a better History and Physical examination and that can not be performed over the internet

Stomach blockage, ulcer, appendicitis, kidney problems... there's a list as long as my arm, he needs a doctor, not a handful of strange internet users trying to diagnose him.

If the G.I. system cannot get rid of stuff the normal way, it comes back up. If there is "normal" functioning between bouts, it is not complete blockage. Get that colon cleaned out before it kills him, or he needs to be cut in half and "resected"

I'm not sure which side it's on, but it could be your appendix. Or it could be twisted bowels or a hernia. You should see a doctor.
