go to PAN option
Enter your details and get a print out of the PAN proof
Or if you have the xerox of the PAN card then just fill up the form available at the above mentioned site and deposit it at any UTI service center.
You will have to pay some charges for the issue of the new PAN Card

It should certainly be not in order to apply for a new PAN Card, as though the applicant is a different assessee, where a card has already been issued. It is because it will disable the authorities from taking appropriate preventing action against abuse of lost card falling into wrong hands.
The reader has raised a common question in respect of a lost card, which should arise in quite a few cases in future.
There is as yet no procedure indicated either in law or any of the instructions.
It stands to reason that a duplicate PAN Card should be possible in such cases. It is better that the Central Board of Direct Taxes comes out with necessary instructions on the subject for the guidance not only of the assessees but also the officials themselves.

There is a simple form to be filled up in blue color, to get a duplicate PAN card. You require to attach a zerox copy of your original lost PAN card. You can get the form at any IT office or at UTI bank / branches

Very sad. There is misc. expenses for duplicate.
1st of all go to police and report the matter. They obtain proof of PAN and apply.

No provision for a duplicate card. Instead, you have to lodge a complaint in your jurisdictional police station with this regard with card details. Then you can apply for a fresh one, as earlier you applied.
