Call your dr telll them you want a copy of your medical records usually they make you sign an authorization form to release them. anyone cannot view them w/out your knowledge.

You can call the medical office and ask for your medical records. You own them and they will ask you to sign a release form. It covers all confidentiality acts.

CALL THE PLACE OR PLACES WHERE YOU HAD MEDICAL ATTENTION AND GET THEM TO FORWARD A COPY OF YOUR RECORD TO YOU OR ANOTHER MEDICAL FACILITY.. THEN ONCE EVERY RECORD IS ASSEMBLED TOGETHER GO READ IT OR GET A COPY OF IT. SAME WITH DENTAL ETC. IF THE FACILITIES ARE CLOSE-BY GO IN PERSON TO PICK THEM UP. CALL FIRST
