That is called a lose/lose situation.

If you tell the truth and it is a negative opinion start looking for a new job because your boss will find out what you stated.

If your opinion is a positive opinion, if your Boss's boss disagrees, then look for a new job, and/or be labeled a brown nosier.

Hmmm, accidentally loose the feed back form and hope they do not send you another one.

Been there done that! What does management put their employee's in this position?

Can you give anomalous feedback ?

It really depends on how bad is your boss. If he is good, I don't think you will ask this question.

If he sucks, then you have to know what can be the consequences if your feedback is not confidential.

If you care about this job, I will say middle of the road responds. If everyone in your company knows your boss sucks and hated him, then you can be honest. Otherwise you will be the one who cover up your boss or kiss his ***.

your boss"s boss is very intelligent to asses your boss, don't undermine your boss, no back biting/stabbing, u r no judge to sit on the judgement of your own boss. otherwise u would have been in his place......this is dirty game.. politics... keep away

This has to be an honest expression and should be frank to give the right feedback. Before giving opininon that there is no retribution that is contemplated post your feedback.

You have to be a bit diplomatic.
Accentuate his positive qualities.
Downplay (but must state)his negative qualities.
I have done that under "areas of improvement"
