Transvestite is an offensive term, yes. The generally preferred term is crossdresser, but this too can offend some folks.

It is not, unless the person asking is not appropriating with the specified problem. If you have not experienced the said term, you are not one to judge the validity of whether or not it is a bad term. You must, in a sense, not pass judgement unless you qualify to judge i.e. apply to the specific term.

transvestite is not vulgar.. "shemale" is vulgar..
shemale - generally used to define male-to-female porn stars or prostitutes
transvestite - a man who becomes sexually aroused when dressed in women's attire
transgender- someone who does not agree with the "gender role" society has assigned (ie a male who does not like to be boyish or manly)
transsexual - someone who desires to live their life in the opposite sex to that which they were biologically born.
what causes it?? good question. the experts are divided.. kind of that entire nature vs nurture debate
personally, i am leaning towards nurture.. i consider myself more unhappy with my gender role that with my sex.
i was raised to be a kind, soft, sweet boy. i was allowed to cry and express my heart on my sleave. i always craved romance and intimacy (as opposed to raw sex). i feel much more comfortable chatting with a table of girls (even though i sorta stick out) thn i do with a table of guys.
many girls have told me i am like their best "girl" friend. as in best friend..
i've also suffered rejection from women, so there is an angry side of me that wants to become a woman so i can show the world how a woman really should be!
i also think biology comes in to play.. as far as sex goes, i just don't feel comfortable putting my penis in a vagina.. tried to do it a few times, couldn't. felty uncomfortable.. but i loved giving oral to the women i dated, and i desire to try giving oral to a man..
the only way to keep me arroused is if i have gentle anal or nipple stimulation while i masturbate..
so yah, i don't see comfortable with the male sex role either... hence, my desire to become a woman.

No the term is not vulgar and it is not a condition but your ignorance obviously is.
I'd get that looked at if I were you.
