An Indian medicine bag is just a bag with important items in it. It can contain anything from sage or sweetgrass to an animal tooth or bone (any any number of other things). There is no official bag that is used for these sorts of things, so it neither has to be leather (I have one that I use which is made out of a quilting fabric) nor does it have to be pear-shaped. A medicine bag is so-called due to the medicine put into it, not the shape or material the bag is made out of.

Take two pieces of leather, cut them to the shape you want. In this case we will use a Pear shape. Simply sew almost all the way around the edges of the two pieces after matching them together. Leave the small end open. After sewing, turn the bag inside out. This will hide much of the stitching. Put whatever items of significance inside. Close top with a leather thong.

omg i learned that in school um with a big peice of brown aged paperbag and form it into a bag
