You need your birth certificate and proof of citizenship I believe if you were not born in the USA. Today all children have to have one shortly after they are born, if you have one and have lost the number, you can request a new one by the same means as initially applying. Go to the nearest Soc Security office and fill out the application. To find the nearest Soc Security office, look in the phone book under United States Government.
