This really all depends on your insurance company. With my 2nd child I had to pay an estimated amount that my insurance would not cover by my 4th month, just for the OBGYN. After birth I got bills from the hospital and whatever the insurance did not cover, including co-payment for each visit after the first visit.
I am expecting my 3rd child in May and have already had to pay the estimated amount, plus 3 co-pays that have been applied to the bill already.

My son, two 1/2 yrs ago, I went hardly ever - maybe once every two months or so, not often, and they really didn't do much, and I paid only ONCE a co-pay of $10... that was it for the whole pregnancy...
But my daughter, one and a half years ago (same dr., same insurance) I went rarely also, but they do make you go every week during that last stretch... but I had to pay my co-payment every time I went. Go figure...
BTW I didn't have a "normal" pregnancy (short cervix & cerclage), but all other visits were to a specialist, so I didn't count those visits or costs in my answer above.

First tri - second tri was once a month. Starting the third tri every 2 weeks, then closer towards my due date once a week. It could also vary by Dr and location. Both pregnancies Ive seen the same OBGYN.
I had to make a deposit on my first visit as I had an emergency c-section with my first son and wont be doing a VBAC. The rest of our deductible is due before I start the Third tri.
Both of these questions could vary by Dr, Id call your Dr and ask them how they do things. It can also vary by what type of health insurance you have.

I went once a month unti my 7th month then I go every week. I only had to pay a co-pay once and my insurance company pays the rest of the visits at 100% after wards. Your ultrasounds and delvery are covered minus your deductible....Check with your carrier....

Once a month. Toward the end, twice a month
My health insurance covers those costs. They will normally ask you when you check in
They will send a bill through the mail, at least they did for me, as far as delivery

I went once per month until the end. After the first trimester, I was set up on a payment schedule & had it all payed for (room, maternity ward, the whole deal) by well before my due date. It's a smart way to go-after the kid is born, you don't have any money to throw away!

Normal visits are every month until the last two months when they change to every two weeks. Co-payments are charged at the beginning of every visit. You have to pay them before you see the Dr.

I went to the ob/gyn every month for the first and second trimester and then every other week for the third trimester up til my last month...then I went every week. They asked for co-pay everytime I went.

They schedule me an appointment once a month, I think that is the norm. I don't know about the insurance because I have medi-cal.
