Why is it that when I go to a shop to exchange or refund something I'm asked for my name and address?

It's something that's becoming more prevalent among retailers. Stores use the information to prevent people from abusing return policies.

for their customer records and inventory also to track supplier's performance .
they need to know who buys the item and why the item is returned or how many times it happened then they will find out is it defected part from supplier or is it the store's fault or just so happen you change your mind.

I think they keep a record on how many times you've returned to that store.
