I think a good title would be "One in Four".
I am not sure on the charm bracelet. Maybe they took one charm bracelet and divided it up and each girl gets something to remember the other three by. Find something that there is four of in the world that every can identify with. Perhaps the seasons or the cardinal directions on a compass.

I like A.Mercer's title "One in Four" it kind of sounds like a statistic like 1 in 4 people have HIV(just an example). Also many people only hear of HIV and AIDS through statistical information so this would give a more personal view of someone that has HIV that the reader can relate to.
