I think you should pee first because it's not healthy to hold it. Quit smoking !!!! That too is unhealthy :)

Go to the bathroom and pee and smoke one fast while sitting on the toilet lol. Then dump it in the toilet and flush it down. You kill two birds with one stone :) Or you could just quit smoking and live a little longer :)

So, let me get this straight: you're asking if you should strain and/or possibly damage your kidneys and bladder by holding in your pee so that you can fill your lungs with smoke and toxins, thereby damaging them. Does that sound about right?
