China was once banned from the world cup as most chinese people look same and players couldnt be differentiated by face descriptions (that's what they thought and some of us also agree), so the chinese team actually brought in new different players (without informing the authorities) after half time to play fresh! the officials werent blind enough and they noticed that and hence china was banned for a while from the world cup by FIFA.

Iraq was banned for the next world cup after Saddam's son tortured team members for losing in that world cup.
R9: Juventes is a club team. Only national teams go to the actual World Cup.

there has been teams banned temporarily(suspended is the most correct term) for not going by certain regulations
including recently teams like Kenya, Greece, Macau and Myanmar
Iran was until recently suspended
Costa Rica was suspended for a while back in the late 40's
EDIT
if a nation is suspended is usually because the national governament or a national team did something
not because of what a particular club did
Juventus was suspended, not the Italian national team(they are 2 separate institutions)

Not from the Cup recently but Juventes in Italy was stripped of two titles in a match fixing scandal last year.
FIFA is made up not just of countries but of clubs from the various leagues around the world and FIFA has quite a few world cups played. The club world cup just finished recently in Japan.
