I would say it's egos and the development of roles within the team or work group. Often times the alpha personalities decide the outcome for the entire group. A person that may not be the most charismatic or outspoken might hold the key to success for the entire group. If their role within the group has been decided by the alpha types then their contributions will be minimal.

People who don't trust others to do the job effectively, control freaks, lazy slackers who refuse to pull their weight, "forge-aheaders" who refuse to read directions or listen to others, the guy who thinks everyone is out to get him, the pair playing footsie under the table,inconsistent leaders in it only for themselves... in a word (two actually) human frailty. Sad but true.
