look the person in the eyes and say those words and nothing else except meaning it when you say it. do not try to make excuses of any kind. no rationalization. Two words, I'm sorry.
If you truly value the person, you might want to add that, but be careful to not say too much. Short and sweet is the best way.
If you are only partially at fault (which is usually the case), wait out the other person, give them time to see the err of their ways, and if you feel you've been patient and they haven't replied in kind, ditch them and get better friends.
You shouldn't be a doormat for your friend.

Say it and then show that you mean it. And remember in every relationship you sometimes have to give more than you get.There are times when you might feel wronged but have to take the initiative. But it has to go both ways, don't be a door mar either.
