Replace on a case by case basis. If you examine it and it is in strong condition with no cracks or creases, then you don't need to replace it. Always take a close look at your belts and hoses when you check your oil. At the first sign of cracks or creases replace them. Also, if certain hoses or belts seem to be losing flexibility, this could also be a sign of impending failure. Belts and hoses are made for the long haul nowadays, so don't just run out and replace for the sake of replacing.

Yes and no. If you look close at it, I bet you will see small cracks in it and definitely replace it. But keep the spare in the trunk. This is insurance like a spare tire, in case the new one were to break for whatever reason and not leave you stranded.

Belts look fine from a distance but you need to get REAL close to give it a visual inspection.

Replace it. They are only about $20. If the old one is in really good shape, chuck it in the fenderwell in your trunk for a rainy day emergency backup. If there isn't a belt routing chart (decal) under your hood, make a note of the path of that serpentine belt before you take it off.

Though you say it looks fine, you should have it replaced soon. Any small crack in the belt can easily be ripped in a snap of a finger while your driving. It is always better to be safe then sorry.
