It's a story line. There are rarely any real rivals in wrestling (Bret Hart and Shawn Michaels were part of that exception).

Just a story line, the ladder thing was an accident, (obviously) and they played it into a story line, there are a lot of people who don't get along backstage, but they keep their differences and opinions to themselves, they know it's best for their part in the business, if things do get out of control however, that's when a lot of wrestlers get moved to other shows, for example, the reason Bobby Lashley is on ECW now is because he wasn't getting along with the writer on Smackdown, he didn't like the guy at all and got into a few heated arguments backstage, this happens every once in a while, but in the back it's mainly little things that everyone just keeps to themselves about what they don't like about someone, Randy Orton made one of the biggest scenes backstage when he was suspended for telling the women how great he was backstage and some talked to Vince about him harrassing them, they said he acts as if he is Gods gift to women and that he is extremely egotistical, though he had been warned about that attitude before without suspension, this time they had no choice but to let him know that he can be out of the company just as fast as he came into it.

It's just story line. Like all feuds it's just writing. Many of these guys are friends in reality.
