I already know how to refinish the tub if I decide to do that but I do not know how to patch the hole.

you need to things, a putty-metal stick and then porcelan touch-up kit. Both can be found at any hardware store. The metal repair is a putty-like substance that you mix with your fingers, squeezing it. Then push into hole, dries fast, sand a little, then use porcelan repair to paint over it. should be easy.

If you access the hole and suitable tool, you can unscrew the sleeve and replace. If it's the screw type you can patch with plastic steel putty that goes by the brand name "Devcon" which you can purchase from hardware store. The set comes in two containers the putty in 500gm tin can and the hardener in glass bottle. You mix both on metal plate or even surface with scraper to the prescribed ratio until the putty is tacky and suitable to apply to the hole. Do not touch the compound with your hands. The chemical reaction generates high temperature. When you have applied the compound to the desired area leave for 30 minutes to let it dry. Have it a go. Bye.
On second thought if the product is not available in you area, you can try "Thistle bond" which are resin with hardener. You mix to prescribed ratio as above. But this product is applied with fibre cloth tape wound around the pipe and apply the resin as you go each layer as you would building an FRP boat, Let it stay to dry, harden when dry. Happy try.

I tried fixing the problem with matching colour and it did not look right to me, so I bought a beautiful deep yellow plastic paint ( a tiny can) and I painted a flower on and around the rusty area. I covered the drain with a plastic drain cover with small holes (from a dollar store) and presto--everyone admires my new sink.

I think they make a Porcelain repair kit. It includes a bonding agent and coating. Depending on the size of the hole this may help. Try this
