Why aren't you comfortable with it? Are you not a friendly, outgoing personality type?
Then do it, just to keep your job and earn some tips.
If a waitress greeted me that way, I'd say "hi ****", and smile. Might or might not tell her my name. Don't like my name.

Wait staff getting chummy with customers is something that began in the late 70's. Fortunately it does not happen in better restaurants. It is one thing for the server to introduce himself, quite another for the server to ask the customer's name.
I say you have three options.
1) Order
2) Order and if you don't like the waiter's forwardness tell the manager afterwards.
3) If you think he's goodlooking and you want to go out with him: tell him your name, get his nunber, give him your order, make sure he attends you well, leave him a nice tip.

I don't enjoy creating controversy or problems just for the sake of confrontation. I truly do not care if a waitperson asks for my name. I'd tell them. Why would anyone be upset at being asked your name?

I would just say Hi and then start ordering my food.
The server may just have wanted your name so he could address you more personally then just saying ma'am.
.....or maybe he just thought you were hot! LOL :o)
