First, you should know and well acquainted with your topic, second you should have the summary from the topic, third, your topic should be within the context of your survey... and by then you are ready for your introduction as it will just be the subject matter on hand of your Reseach for the preparation of the readers of what is it is all about. A three to four paragraphs will do.

Do you mean an intro to motivate the survey responders or is it for the people who will read the results after the survey? Assuming the latter, just tell us what the problem is, why it is important and how your survey can reveal (or did reveal) areas for improvement. You might even tease the readers a little bit if you can mention that some of the results were completely unexpected.

what intro? u mean the instructions? that depends on what type of survey are you using.
