It was all a test for his 'sidekick' (cant remember her name) to see if she was strong enough to continue after his death - all went horribly wrong as you saw lol

The sidekick (Amanda) was being tested for leaving her past and starting a new life because she passed his test. She failed the test because she was a murderer and shot the Doctor, without knowing that the guy she was monitoring all along the movie is the doctor's husband. The husband shot Amanda, and when he saw what was happening to his wife he was so angry at Jigsaw/John that he decided to kill him (even though he said "I forgive you"). Since the doctor/wife's torture device was connected to John's heartbeat monitor, she died. John pressed a tape player and the tape said to the husband something like this "Since you didn't learn anything from the test you went through all movie long, I'm glad to tell you that your daughter is trapped in an air confined space and I'm the only one who has a key and know where she is, so your screwed". The End.

Okay, you know the guy, Jeff, who's son was killed by a car? He's the one that was going through all of the trials, with the ice woman, and the pigs, and so on.
He's actually Lynn, the doctor's, husband. You just don't know it, because in the beginning of the movie, she's with another guy. Well, she's cheating on her husband with this other guy. So the little boy that got killed was her son, too.
Apparently, Jigsaw set the entire thing up as a test for Amanda, his apprentice, only she didn't know it. She didn't pass the test. Amanda shoots Lynn, and Jeff shoots Amanda. Then Jeff kills Jigsaw. When his heart rate stopped, it caused Jeff's wife to blow up, because her trap was linked to the heart monitor. Right before Jigsaw dies, we learn that he has Jeff and Lynn's daughter, and he was the only key to finding out where she is. So, no one learns anything, and we're left with a bloody mess.

Lynn succeeds in keeping John alive, but during the course of operations he semi-consciously audibly professes his love for his old girlfriend Jill during a dream. Amanda interprets his comments as being towards Lynn, and in jealous anger does not follow his instructions to remove the collar after she completes her task. When Jeff reaches Jigsaw's room, Amanda shoots and wounds Lynn while Jeff fatally shoots Amanda in the neck. In her final moments, John expresses his disappointment with her over making her traps unwinnable, and that this was in fact a test designed for her. Her jealousy killed her, because by attacking Lynn she provoked Jeff to deliver a fatal blow. Last, Jigsaw reveals that Jeff and Lynn were in fact married. Finally, John offers Jeff the chance to forgive him. Jeff opts instead to slash John's throat. Before he dies, John plays a tape explaining the details of Lynn's trap, explaining how he has effectively killed her, and that he has trapped his daughter in a room with a limited air supply and that he will have to play another game to find her. In short order the doors of the room seal, Lynn's collar kills her and Jigsaw dies, leaving Jeff screaming alone. The film ends with a quick montage of all the traps seen thus far in the Saw series.

Yeah, it was a test for the girl (his sidekick). The entire time she thought they were testing the married couple but he was really testing her. He believed that her games were never fair and only led to death in all cases. Anyway, she fails this test so she dies and also the doctor was only to be kept alive as long as the old man was kept alive so it was also a test for her husband. He thought he was doing his wife a favor by murdering the old man instead he killed his wife, therefore he became a murderer as well.

I only saw the ending, and basically what i got was :
it recapped ALL 3 fraggin movies (YES, it flashbacked itself!!!)
revealed the girl from 2, was actually the "Jigsaw" apprentice of one, was basically a heartles b****, and Tobin Bell was sick of her crap and made this final shindig a test to see if she was "worthy" of being Jigsaw or just a cold hearted killer.
she was a coldhearted killer, she dies, everyone appears to have died.
yippee, now we get to look forward to Saw 4, the beginning and "The Jigsaw Puppet: takes on Manhattan!"

Have not yet seen this one BUT it is most likely a cliffhanger for Saw 4 for Halloween. I will be buying it sometime today though.
