Everything, including people in a 5 mile radius would literally go up in smoke, leaving a glass-lined crater. From 5 to 10 miles there would be a field of total rubble, with few building materials being recognizable.From 10 to 15 miles you would begin to recognize shapes as once having been buildings. There might be a few cars and trucks lodged against them. Manhattan Island is only 24 miles long. Recognizable body parts and corpses would probably begin to show at 15 miles Any survivors within this radius will likely not live out the day. What the shock wave does not destroy, the firestorm will set ablaze. Firefighters from outside the city would have to fight their way in.
A city the size of NYC just might have 1,000,000 survivors. There's much more ugliness, but that's about as far as I want to go with it.

You'll have a 10 mile radius of complete devistation, and partial destruction out to 20 miles or more. Then the fallout will follow the pattern of the prevailing winds and begin to drop to the earth as radioactive ash that can sicken and kill. A metropolitan area would suffer near 100% casulties within the ten mile radius, and at least 50% within the 20 mile radius. Depending on the fallout pattern, thousands of people, perhaps tens or hundreds of thousands exposed to the fallout will sicken and die horrible deaths within the next few days to weeks.
Not something I'd wanna risk if I was the president of Iran...
But hey, if he's got his heart set on it, I'm sure the Isrealies will oblige and drop a few on him.
