It means that human nature is not inherently good. In other words, do you have to teach your children to lie to you, or do they tell the truth first, and you teach them how to lie? Very much the opposite. The first situation is always the case, meaning, human nature is to look out for itself, any way that it can. Demented is a distorted outlook on reality, so by saying that statement means that your human nature, besides allready looking out for itself and doing any means to do so, will have a distorted view of society and life in general in the process.
I have heard it before, as it is a common phrase, and spoken by someone you know, can just mean that they don't agree with you. However, hearing that from your psychologist might mean something isn't quite right.

It means that the person is as demented/crazy/evil as what he/she is thinking or what is the output of his/her brain.

This simply means when you've got twisted thoughts in your mind, motives to commit acts of violence, murder what have you.
Meaning everyone has a dark side, sometimes its all we focus on when we want revenge.
