What causes that random beeping sound you hear in your ear?

Some people say that it means someone is talking about you, but I would like the actual scientific explanation.

sometimes it is accompanied by hearing loss. I know someone who has moderate hearing loss and constant ringing in her ears. the doctor says its all one package
