He originated from Amazing Fantasy number 15. He was the result of a spider bite from a radioactive spider during a science exhibition.

I believe he was an original creation by Stan "The Man" Lee and Jack Kirby. His original appearance was in a comic called "Amazing Fantasy" issue 15 if memory serves. After that, the comic stopped being "Amazing Fantasy" and became "The Amazing Spiderman.
