I really wouldn't know how to do it yourself, but as for getting a professional look, i'd suggest Destroyed Denim. destroyedbrand.com its owned by Christine Dolce aka Forbidden from Myspace; you might be a bit skeptical, but i've sent a pair of my jeans there, and believe me, its definitely worth it :D

get relli good sandpaper and sand down the area you want to destroy..when you see strips of fiber ...stop sanding and tear the jeans a little at a time until you reach the width of the tear...sandpaper also helps you fade out the areas you want ..too

Use scissors and cut little or larger snips in your jeans. Then wash them. Worked good for me.

MY Friend uses a cheese grater to shread her pants and sandpaper and if you want them to fade, bleach 'em
