It's surprising that nobody else has suggested this. Probiotics. These preparations contain large amounts of lactobacillus acidophilus and if they also contain lactobacillus bifidus, so much the better.
One buys these as capsules or tiny envelopes of powder in the refrigerated section of the health food store or pharmacy. Each capsule or envelope should contain a minimum of 10 billion live lactobacilli cells. Check the date stamp - product should be fresh.
Where I live, one company manufactures edible portions in tiny one-ounce bottles that look and taste exactly like yoghurt, except that one bottle contains 50 billion cells, or as much l.acidophilus and l.bifidus as 10 quarts of regular yoghurt with live lactobacilli.
I was directed to these probiotics years ago by my young son's pediatrician. The child had small cracks at the corners of his mouth, not cold sores. The problem cleared up perfectly.
Years ago I had a friend who used to get cold sores on his mouth. In his case it was a reaction to strong sunlight. He was one of those people who complain about their ailments, then when somebody proposes an inexpensive simple remedy they moan on about O, No, It'll Never Work I've Tried Everything I Just Have To Suffer. Eventually, though, he broke down and bought some probiotic preparation. It worked perfectly. He was enchanted. Forever after, whenever he felt a cold sore coming on - apparently the outbreak is preceded by a tingling sensation in the area - he would stop it cold turkey with a couple of days of probiotic capsules.
These preparations are not expensive. One takes them on an empty stomach. To begin with, something like one in mid-morning (don't eat right away) and a second one in mid-afternoon or before bed. One could increase the dose to 3 per day.
Probiotics are beneficial for a lot of complaints, including certain viral and yeast infections - and your cold sore is a Herpes I viral infection - plus they are helpful for countless gastro-intestinal disorders and they are said to even lower LDL, the "bad" cholesterol.
They are not drugs, they're nutraceuticals. There's a lot of research in this area.
Good luck to you, and hope this might be of some help.

Cold sores are viral. Like some of the other people have mentioned, you have to be VERY careful to not let any of the sore or its oozings onto any open wound or mucous membrane (mouth, eyes, etc). This also applies to other people's bodies, this is contagious.
That being said, if you can afford it, Melissa (Lemon Balm) oil applied topically should help. This is the oil, not the tincture. It's fairly stinky, but effective. It is also said to be effective on the pro-domes (bumps that will turn into cold sores).
After you get over this outbreak, and especially if you have sores frequently...
Take lysine (an amino acid), especially when you feel one coming. Try to avoid foods containing arginine (another amino acid). See if they are triggered by stress or an excess of caffiene, and try to change the situation.

Your best bet is going to be getting some medication from your doctor.
In the mean time, keep it covered with one of the creams from the store. If you allow it to get dry, it can crack, which allows the oils to spread, and allows other stuff to potentially get in and infect it.
Saltwater and mouthwash will generally have no effect, except to clean it, which is certainly not a bad thing.

Did you try Abreva? It shortens the length of the time you have the sore significantly. All the other products on the market just bascially dry out your lips. Unfortunately there is no "cure" for a cold sore, b/c it's caused by a virus. Just don't pick at it, and wait it out. And don't touch your eyes or other, um, moist, parts of your body after you've touched it.

You can make his formula by buying bulk herbs. Use only organic or wildcrafted and make a tincture..Take internally swishing around mouth after you make a tincture (very easy to do..I'll get you directions. Use goldenseal, garlic, and scullcap. May use fresh garlic in the tincture.
EDIT--although I still kept how to make it..I found a website that sells the tincture for only $1.91
http://www.techcheminc.com/dr_christophers.htm It is called CSR (cold sore relief) under the heading christopher's personal products.
