considering that my neck size is average, but small chest 37".
Always have trouble finding shirts that actually fit comfortably

Most upscale men's clothings stores or departments will alter shirts from the floor to suit you.

I believe landsend.com also sells custom mens and womens clothing.

Are you looking for big/tall, or just custom?

You should check out Ike Behar stuff (http://ikebehar.com/). You can get his stuff in stores, or custom made from his website. You'll need to go to a shop and get your measurements (any tux shop will do it for free). My husband has a few shirts by them and loves them.

Yes, go to a high end clothier. They could either tailor an existing shirt or start from scratch to make it fit you.
