Generally editors and copy editors are responsible, especially at the larger publications. However, reporters are often encouraged, especially in smaller publications, to suggest their own headlines.
I work for a series of Town weeklies on Long Island and most of my suggested headlines run, although at least two editors have the final decision.

When I was a newspaper reporter, my editor always created the headlines. He picked the headline that would fit best, depending on where he was going to physically place my story on the page.

Reporters may suggest headlines for their stories, but the copy editor writes or approves the head.

The editor decides which stories appear where and the sub editor puts in the head lines. If the editor has an idea for a head line then they can override but generally the sub editor will decide...
