Do you really mean "disqualified"? This can happen for many reasons depending upon the level of competition: Not being properly entered into the event; not showing up on time for the beginning of competition; entered into too many events; not wearing a proper uniform; unsportsmanlike conduct; wearing unacceptable jewelry,
Caught using sticky adhesives on hands, illegal taping on hand or fingers; using unapproved or underweight disc.......more

If you meant , "Foul";This can ocurr:
1.If during an attempt, the competitor touches the inside boundary of the circle or top of toeboard (or top of metal boundary circle, if that type is being used).
2.If during an attempt, any part of the competitor or his/her clothing touches outside of the throwing ring.
3.Exceedes the time limit allowed before throw is completed.
4.Disc lands out of sector
5. After throw, competitor walks (or falls) out of the front half of the circle.
Also, in some competition, disc will not be measured unless it exceeds a given distance.

A foul in discus occurs when
1) a competitor fails to enter the circle from the rear
2) a competitor fails to exit the circle from the rear
3) a competitor exits the circle prior to throwing the discus
4) the discus lands out of bounds
5) the competitor tests positive for banned substances

if you step out of or onto the circle before the discus lands and you have regained balance, it is a foul. also, if teh discus lands outside of the marked region, it is a foul. obviously, if you take steriods or something stupid like that you could get disqualified too
