Each parent has custody of one child.
Father pays for health coverage for the child he has,
Mother pays for health coverage for the child she has.

Divorce decree states each parent pays for the health coverage for each child they have.

Under the Extraordinary Health Care Expenses it states:
Father pays 100% for his child
Mother pays 100% for hers.

Is this saying that if the child that resides with the father has to have major surgery, the father has to pay 100% of the costs? Or does the costs get divided according to their respective incomes as does all other expenses like child support, travel, daycare?

You pay for yours completely, she pays for hers completely.

100% means 100%. There are no reparations in the decree which account for income unless it was written in the decree. If it states that each parent will pay 100% for each child that resides in their custody, that is what it means. One can always have a meeting of the minds with the other parent a child needs surgery and the custodial parent can't afford it, and ask for help. There is no law against that.

Good luck.

Without seeing the other provisions in the decree, the two provisions seem to say each parent pays 100% of any costs associated with the child in his/her custody. I don't see how it can be interpreted any other way (unless there's some fine print later on that negates these provisions in some way, or if your state has some sort of law that prohibits a division such as that).

If you have custody it's your responsibility unless you have made other agreements with the mother
