i think in this case it is
a CPA
a MD
however it does sound better as a cpa & an md
usually 'an' is only used before a word beginning with a vowel
for example,
i couldn't believe she bought an ugly coat
instead of
i couldn't believe she bought a ugly coat

As you would be pronouncing the letters (not the words which they represent) in this case it would be:
a CPA
an MD
